GETTING STARTED
WHAT'S THE FIRST STEP?
Once you contact Dr. Jablonka, she will set up a brief, free, and confidential consultation call to discuss your concerns and to determine if she is a good fit for your needs. At that time, you can also set up an initial 90-minute intake appointment via tele-health. Prior to your intake, you will receive an intake packet to review and complete.
WHAT HAPPENS IN THE FIRST APPOINTMENT?
During the intake appointment, Dr. Jablonka will conduct a comprehensive assessment to better understand your difficulties and to discuss treatment goals. From there, Dr. Jablonka will provide treatment recommendations, as well as referrals if necessary.
HOW OFTEN ARE SESSIONS HELD?
Sessions are generally held weekly for 45 minutes. All sessions are conducted via tele-health using a secure video-conferencing platform.
HOW LONG DOES TREATMENT LAST?
While evidence-based treatment is generally time-limited, treatment duration may vary. In formulating a treatment plan, Dr. Jablonka will provide you with an expected length of treatment. Additionally, Dr. Jablonka will monitor your symptoms and regularly provide feedback over the course of treatment.
DOES DR. JABLONKA ACCEPT INSURANCE?
Dr. Jablonka does not accept any insurance. Some insurance plans include out-of-network benefits that may allow you to submit claims for reimbursement. Please check with your insurance provider prior to the start of treatment to clarify your specific behavioral health coverage, specifically in regard to tele-health. Dr. Jablonka will provide you with a receipt of payment each month with all the necessary information you need to submit to your insurance company for reimbursement.

WHAT FORM OF PAYMENT DO YOU ACCEPT?
Payment is expected at the time of service and only via credit card.
WHAT IS YOUR RESCHEDULING AND CANCELLATION POLICY?
Sessions cancelled without 24 hours advance notice will be charged in full. If you need to reschedule an appointment, please provide as much notice as possible.
